The Hälsa Membership Plan

There are two plans to choose from, depending on your dental needs:

THE FINE PRINT:

Additional family members must be registered at the same time as initiating family member in order to get additional family member rate. Family members eligible for savings include spouse/partner, and dependent children under the age of 26. All savings plan membership fees are due and payable in full at the time of registration. All savings plan membership fees are NON-REFUNDABLE. All additional charges are due the day of service. Savings plan duration is for one calendar year, starting on the registration date. Savings plan coverage expires at the end of the membership year. A $100 fee may be charged for broken appointments without 48 hours’ notice. The plan may be cancelled and no refund granted after three broken appointments without adequate notice.

EXCLUSIONS AND LIMITATIONS:

Savings plan members cannot use any other dental benefits with this plan. Additional discounts (senior, cash payment) cannot be combined with this plan. Using Care Credit to pay patient balance reduces 20% discount to 15%. The savings plan will not cover any portion of a specialist referral. The savings plan will not apply to costs of dental care for injuries covered under workman’s compensation. The savings plan will not apply to dental care for which in the sole opinion of the treating dentist lies outside their scope of care.  Discount does not apply to SureSmile clear aligner therapy or any in-office product purchase, including whitening treatment/product.
